Job Summary
We are seeking compassionate, reliable, and dedicated Licensed Practical Nurses (LPNs) or Registered Nurses (RNs) to provide skilled in-home care for an adult individual requiring 24-hour support. We are currently hiring for all shifts, including days, evenings, nights, weekdays, weekends, and holiday coverage.
The ideal candidate will be comfortable performing both skilled nursing duties and personal caregiving tasks in a private home setting. This role offers the opportunity to make a meaningful difference by providing one-on-one care in a safe, comfortable, and supportive environment.
About the Individual
The individual receiving care:
Has Muscular Dystrophy
Is ventilator-dependent and has a tracheostomy tube
Requires 24-hour in-home care and supervision
Uses a motorized wheelchair
Has limited range of motion but is able to communicate verbally
Resides in a safe, welcoming community and comfortable home environment
Responsibilities
Skilled Nursing Duties
Perform required skilled nursing interventions throughout each shift
Monitor respiratory status and overall well-being
Provide tracheostomy care and suctioning as needed
Assist with ventilator-related care
Document care and communicate any concerns or changes in condition
Personal Care and Daily Living Assistance
Assist with medicated nebulizer treatments
Provide sponge baths and personal hygiene assistance
Assist with clothing changes
Support toileting needs
Comb and groom hair
Assist with oral care, including brushing teeth
Reposition and provide comfort measures as needed
Maintain a clean, safe, and supportive environment
Qualifications
Required:
Current Wisconsin LPN or RN license in good standing
CPR certification (or ability to obtain)
Strong communication and interpersonal skills
Ability to work independently in a home-care setting
Compassionate, dependable, and patient-centered approach to care
Preferred:
Experience with ventilators and tracheostomy care
Prior home health or private-duty nursing experience
Experience caring for individuals with neuromuscular disorders
